Many factors
influence the longevity of dental restorations; therefore
we can only give our best estimate based on
the extensive skill, knowledge and judgment of
Dr. Fawcett. Because we believe investing in your medical and psychological
well-being is a wise, long-term decision, we are confident
in providing you a unique warranty on the work we do.
WE
WILL GUARANTEE YOUR DENTAL RESTORATIONS FOR 5 YEARS *, Here’s
how it works:
Our Part:
If our crowns, veneers, or onlays require repair or
replacement within the first 5 years for any reason
other than decay, and the tooth can still be saved,
and you have fulfilled your part of this agreement
(see below), we will replace
it at NO CHARGE to you.*

Your Part:
All warranties are provided under the following
terms:
| a) |
You keep your regular hygiene
re-care visits at our office at the interval
Dr. Fawcett
has
recommended specifically for you. The number
of cleanings or periodontal maintenance visits
recommended is based on the health of a person’s
gum tissue and can be between 2 to 4 (or more)
times per year. |
| b) |
You have completed, within a one
year period, any treatment recommended by
Dr. Fawcett, for the quadrant in which the affected tooth is in.
(Optimal health
requires a comprehensive approach to care since gum infection and bacterial decay
spread from tooth to tooth.) |
| c) |
You advise us at least 48 hours
in advance of any appointment changes. |
| d) |
If you move out of Texas, and
you have fulfilled your part of this agreement,
you
are only obligated
to visit our office once annually for a cleaning & check-up,
in order to maintain this warranty. |

*Note:
| 1. |
If a minor restoration is damaged to
the extent that a crown is required, the full fee originally
paid for
that restoration will be applied toward the crown fee.
You only pay the difference. |
| 2. |
We cannot predict if or when gum or
nerve treatments may become necessary. If needed, they
would
require
additional fees. |
| 3. |
This warranty does not cover obvious
or intentional abuse or failure to complete recommended
treatment
in our office. |
